Long-Term Condition (LTC) Reviews Update

Due to the Coronavirus outbreak and social distancing measures, we have adapted our Long-Term Condition review system.

Reviews will be carried out by telephone and/or online wherever possible. We will advise if face-2-face appointments are required and try to keep the number of these to a minimum.

Having any current health concerns? – do not wait for your review CONTACT US NOW

If you have regular medication, please look out for a prescription message and attached letter asking you to speak with Reception to make the necessary appointment(s) with the most appropriate member(s) of staff.

Please do NOT book an appointment directly with a GP for this review.

IMPORTANT INFORMATION BELOW

The Practice has a professional duty to ensure that the correct medication is being prescribed to each patient and monitored. Medication which is not monitored can be detrimental and in some cases, life threatening.

If you are not reviewed, we may have to remove your medication from your repeat prescription.
