Practice Care Coordinator (Nurse) Vacancy

22 hours per week – permanent

Pay: negotiable according to experience

Working under direct supervision of the GPs, we are looking for a Practice Care Coordinator to work with our team of GPs, Nurse Practitioners, Nurses and Health Care Support workers.

The aim of this clinical role is to support both the practice staff and members of the Neighbourhood Team to identify and support people to reduce the risk of unplanned hospital admissions and to effectively support those individuals in the community, particularly patients who are frail.

The ideal candidate must be a Registered Nurse with the ability to work autonomously; have knowledge of dealing with people with long term conditions and evidence of working within a multidisciplinary team.

We are also looking for excellent interpersonal and communication skills and be highly motivated in your approach to care delivery to ensure we maintain a high quality service delivered to our patient population.

You must be able to travel as home visiting is required for the post.

Dispensary Assistant / Dispenser

37.5 hours per week (plus holiday / sickness cover) – Permanent

Pay: Competitive according to experience

Working under direct supervision of the Dispensary Manager, we are looking for a reliable and flexible Dispenser / Dispensing Assistant to work within our very busy dispensing department. The pay is dependent upon qualifications and experience. Although a qualified Dispenser is preferable, full training will be given for the right candidate.

We will be looking for the following personal qualities:
